A skilled nursing facility (SNF) offers 24-hour licensed nursing care, catering to individuals who need ongoing medical attention, nursing services, or rehabilitation for injuries, disabilities, or illnesses.

Services provided at a SNF typically include rehabilitation, intravenous therapy, post-surgical stabilization, pulmonary management, wound care, medication administration, and assistance with daily living activities such as dressing, bathing, walking, and eating. Additional services may encompass psychiatric and psychological support, social services, podiatry, optometry, housekeeping, laundry, and beauty/barber services.

1. We review the medical records to determine if we can meet their medical needs:

A. Physician Orders: When someone is interested in coming to the nursing home, we start with Physician Orders stating that the person needs nursing home care. The orders spell out how we are to care for the individual – for example: do they need physical, occupational, or speech therapy, do we need to check their blood pressure, do we need to check their blood sugars and how often. The orders should list their current medications and any routine lab work. Everything we do is under the order of a physician, so this is the starting point.


B. History and Physical: Additionally, we need a current history and physical (H&P). While the Physician Orders tell us how to care for the person today; the H&P tell us what to look out for. For example, they have a history of high blood pressure, or congestive heart failure. This information assists our medical team in caring for your loved one.

2. Then we discuss the financial needs.

A. Private Pay: Are you paying private pay? Do you want a private room?
B. Medicare: If you have had a hospital stay within the past 30 days, we can review to see if you are eligible for skilled care and check your insurance coverage for you.
C. Medicaid: If you are not able to fully pay private pay, then we can check to see if you meet both

i. Medical Necessity, and

ii. Financial Neccissity 

We want you to make the room as homelike as possible.  Please feel free to decorate with pictures from home, favorite bedding and curtains must be treated by Maintenance – but, add a nice homelike touch.  We recommend you bring 7-10 changes of clothing and make sure that everything has your name clearly marked on it.  Try to make sure the clothing is comfortable and easy to work with if you are doing therapy.

You have access to your loved ones 24/7.  Please note that our visiting hours are from 8am to 8pm; however, if someone special is in town, please be sure to stop by.  The door is locked after hours, however, you can call or ring the doorbell and someone will come to the door.  If you are visiting after normal visiting hours, we just ask that if you have a roommate, you visit with your loved one in one of the common areas like the dining room, the front living room, or the therapy room on Hall 4 for after hour visits.
